US Importer FSVP Documentation Checklist

Supplier documents to request for review

  • Product specification, botanical name where relevant, grade/caliber and intended use.
  • Available lot or crop year confirmation, packing and sample approval process.
  • Commercial invoice, packing list, bill of lading and Certificate of Origin when contracted.
  • Phytosanitary certificate, Certificate of Analysis, inspection or other documents when required by the buyer and destination.
  • Facility, process and handling information that the importer or adviser asks to review.

Importer responsibilities

FDA FSVP rules place verification responsibilities on covered US importers. The importer should determine whether FSVP applies, identify the FSVP importer, evaluate hazards and supplier performance, keep required records and confirm any exemptions or modified requirements with qualified counsel or advisers.
